Instructions – Mailing Address Form 8843 :

Who should file Form 8843 ?

International Students (F, J, M, Q) visas, for up to ‘Five years’ and Teachers or Trainees (J, M) visas for up to ‘Two years’,
have to file Form 8843 along with their Federal Tax return 1040NR-EZ.

Should I file Form 8843, even if I had no income in 2009, and did not file Form 1040NR-EZ ?

Yes, Form 8843 needs to be filed even if you do not have income in 2009, and did not have to file Form 1040NR-EZ.

Should I file additional Form 8843 for my dependents with no Income ?

1.No if you are a resident of Canada, Mexico, South Korea, an Indian Student and wants to claim your dependents in Form 1040NR.
You can can apply for ITIN, if your dependent does not have SSN.

2. For all other cases Yes,you should still file Form 8843 for each of your dependents.
The dependent should mail her Form 8843 to the IRS, as a separate package and not with his/her spouse’s tax documents.

Dependent can leave the SSN or ITIN number column blank, if she does not have SSN or ITIN. Dependent should not apply for ITIN.

Documents needed to file Form 8843 :

1. Social Security or ITIN Number

2. Passport with I-94 information

3. Address and phone number of your university if you are a Student, Teacher or Trainee

4. Name and Phone Number of your Dean or Program director if you are a Student, Teacher or Trainee.

Documents to mail the IRS :

1. Completed Form 1040NR-EZ (if applicable)

2. Completed Form 8843

3. W-2 Form(s)
